The Talking Book Center Book Request Form is intended for use by REGISTERED LIBRARY PATRONS. The form may be used by family members or friends requesting books for registered patrons.
A total of TEN (10) books may be ordered with the Book Request form. To order more than ten items, the form must be submitted again. The form may be submitted as many times as needed.

TBC Book Request Form relies on e-mail (electronic mail) to send book requests. Complete confidentiality of e-mail messages cannot be guaranteed by
TBC. While a reasonable effort will be made to guard library patron privacy, please be aware that e-mail is not a totally private means of communication.
Instructions
- For best results, maximize the size of the
TBC Book Request Form in the Windows environment of your computer.
- Form Submitted by: If you are requesting books for a family member or friend, please give your name in this field. You may also include a telephone number in the space if desired.
- E-mail address: If available, please enter your e-mail address.
- Patron Name:
Please give the patron's full name. Completion of this field is required before the request may be submitted.
- Patron ID Number:
Please supply the patron identification number if known.
- Telephone:
A telephone number MUST be entered before the request may be submitted.
- Patron E-mail:
Enter patron's e-mail address if available.
- Mailing Address
The Street address, City and Zip code fields MUST be completed before the request may be submitted.
Materials Requested
- Please supply the book number from the NLS (National Library for the Blind) Catalog if available. A catalog number may be entered without a book title if desired... but please be accurate!
- Preferred Format:
Default value is "Cassette Book". Select button for Braille format if desired.
- Special Notes / Instructions:
Use this space to enter any special instructions or notes to the library staff.
- Click on the SUBMIT REQUEST button to send your book request to the library. The system will immediately return a confirmation of the information submitted to your web browser.
- Click on the CLEAR FORM button to clear the form and start again.
